Category 5 Wheelchair - Tailored Category 5 Wheelchair requires technician set up. Set-Up requires a minimum of 24 hours notice. 

Rental Equipment Credit Card Policy

To streamline the rental process and ensure a smooth transaction, we require customers to have a credit card on file.

Process:

1. Completion of Rental Agreement Form: Customers must complete the Rental Agreement Form before proceeding with the rental. This form includes necessary details about the rental equipment and the customer's information. It is completely digital and will be automatically submitted once complete. 
2. Completion of Wheelchair Build & Set-Up form is required. 

Rentals are not pro-rated. 4 week rentals only.
